Why has the tyre failed that way? Is what concerns me!

Marangoni not a make I would buy (think its a cheap tyre?) Normally stick to better brands.Too much Chinese crap on the market.

Omega is renowned for incorrect camber setting. Resulting in uneven ware across the tread, and in some cases this can cause a blowout or failure as the wear rips through the inside shoulder of the tyre.
If not corrected any new tyre/s may very well go the same way...
...hopefully this is not the case and the tyre has just failed with no other car related influences.
Also hope the incident was not too dramatic....?
