hi there just to let you know the exhaust studs dont go into the water jacket as standard, but i have known of engineers drilling out broken studs, drilling to far into the head and going in to the water jacket 
i've done about a dozen or more heads on v6 engines and its quite rare for the gaskets to fail by them selfs (normal to over heat and warp the heads) its very common for the oil cooler plate to corrode and leak as there is no gasket here just grey sealer or the bridge seals to leak. hope this is helpful for you
