Looks like I may be doing this job soon.

What is the best way to approach it?

I don't have an engine crane too handy.

Is it really possible to to this job with the engine/heads in place? Would you just remove the head?

Thoughts are welcome

I'm having a go at this myself atm.
Assuming you don't get any broken studs/rounded nuts like I have it looks possible.
Engine crane needed to remove OS engine mount though.
I'm stuck with a rounded nut atm, grinding off with the mini grinder is taking ages but its not a rush job.
Front coolant pipe and dipstick tube will need to come off to get the last nut then hopefully I can try and remove the broken stud.
On the plus side after discussing this with MDTM he thinks I may get away with flatting the manifold and using the new gasket if the damaged stud won't play, that or a bracing bar from the other studs.
I'll let you know when I get any further.

Just had mine done with all in situ, the engine mount does need to be removed but the engine can be supporrted from underneath via a good trolley jack. You will need to tilt the engine via the jack. You also need to remove the oil dipstick tube. All the work has to be done from below the car so does get a little tiring but is achievable.
Also you will need to remove the dowhpipe from the cat for access. A man of your calaber........no problem
