Holding back/hesitation

The wife has reported that the Miggy held back/hesitated on her today. I haven't experienced this problem yet but she informs me that it only happened once as she was pulling away from a stationary position.
Any suggestions? My initial thought was maybe a bit of dirty fuel.
I usually fill up with Shell or Esso and the tank still has a 1/4 left.

What transmission is it? What sort of hesitation, slow revs or gear engagement?

It's a manual and it was accelerating away from a standing start in 1st.

i know very little about cars so it'll be interesting to know if someone else says something to do with the fuel injection?

Might be worth checking the plug wells are dry and that there are no codes stored.
