Each dealer will have a parts code, it is a unique number that is given to them to identify them when ordering parts from Vaux.
When you take the form back to the dealer ask them what their parts  dealer code is, they should tell you

its been a while sine i did one but i was under the impression that the dealer could do it for you.

if you are sending it yourself just phone up the parts dept and ask them what their dealer code is, they should tell you over the phone

Yes the dealer has to fill that bit in, plus stamp and sign it iirc.

When they offer to fax it off for you, say ok, but say you'll wait for the form back......otherwise it may just disapear into a backroom....and somebody in a bored moment may just make a paper airplane outa it  ;)
