You've tried new plugs, right?

If not, could be tracking down the outside of plugs 1 & 4.

The spark plugs are new.

There are three wires to the dis module. I believe one is signal for syl 1&4, one for 2&3 and the last one is ground.

I think I'm missing a signal from the ecu. I've measured the signal from the ecu when the engine is running (on 2&3), but I was not able to recognise any problems with a voltmeter. must probably have an oscilloscope.

Is the crankshaft and camshaft sensors the only input sensors that controls the ecu's ignition output?

Try tracing the two signal wires as they go along the cable tray that runs along the length of the engine.

We had a fault on ours that eventually turned out to be a wire-to wire short in this area.  Chafe between the CTS signal wire and injector wiring caused wire-to-wire short, and the CTS to die.  It's just possible you have the same thing going on but with a DISpack wire?

Worth a quick look maybe.  :)
