92 is a little too hot for my liking, never got my 2.2 to go above 90 on the temp gauge, normal running temp should be 85.
I would look at changing the themostat.
Perfect, right thing to do, common issue on 4 pots.
Not good! - Can you hear the fans clicking in?
How does the car start from cold, does it start, but run lumpy and then clear? - Do you have much white smoke out of the exhaust?
Do you have a compression tester? (your son should have one) its worth putting it on each pot and comparing the pressure.
Its not too bad on the 4 pots lots of space to work in. DIY it should cost no more than £150.
Biggest cause of HG failure on 4 pots is either old coolant which has been left too long, or a very weak mix to anti-freeze.
Father tunnies 2.2 which was Vx serviced all its life had the HG fail at 98k, mine is on 130k and its still on its first HG.

In answer to your questions:
1. Yes I think a new thermostat is in order too.
2. Yes the fan cuts in ok.
3. It starts with a lumpy tickover which clears after light revving & runs ok until I have to stop in traffic or park, then the lumpy running returns & the misfire sets in. There is a lot of white smoke/water vapour? from the exhaust which is more noticeable on colder mornings and reduces slightly as the day goes on.
4. No access to tools away from his work unfortunately.HTH
