You need to get the codes read by a TechII to be able to diagnose the problem properly.

Where abouts are you?

sounds like a failed or failing position switch.

try taking it off and drowning it in WD-40 - it's located on the side of the transmission, covered by a thin steel cover. Look for the electrical connection.

Yep. Worth checking that the indicator LEDs by the stick follow its' position accurately without jumping about as these utilise the same switches as the gearbox ECU. :y

While the switch is off, check the linkage is adjusted correctly.

Failing that, check the engine and gearbox ECU for trouble codes. It's not unheard of for an engine problem (e.g. crank/cam sensor) to cause an erratic RPM input to the gearbox ECU which causes it to go into limp-home. Does the engine always start OK, out of interest?

Always worth checking the ATF level and condition, and, if it hasn't ever been changed, drop the sumps and replace the fluid. The fact that it is intermittent and drives OK manually makes me thing it's an electrical issue though.

As already said, selector switch would be a good place to check first. :y

Manual gears can be selected OK as you have chosen which gear to be in whereas in drive the selector has to choose which gear to be in. Will sometimes go into ' limp home' mode, hence the higher gear selection.
